Special Delivery Imaging

Women seeking an elective prenatal ultrasound with Special Delivery Prenatal Imaging require that they are undergoing prenatal care of a physician to use our services. She must have already undergone a Level 1 diagnostic ultrasound to confirm a due date and screening for fetal anomalies or any other pregnancy-related issues. It is the policy of Special Delivery Prenatal Imaging that all women requesting an elective ultrasound are to provide documentation that they are receiving prenatal care. Please have the following form filled out by the attending physician prior to your appointment. You can fax or bring the completed form with you to your session.
